Prestige Assisted Living at Autumn Wind is located on Beech Street between South Kimball and Linden Avenue in an area of newer homes nestled among local farms. The community’s close proximity to medical clinics and West Valley Medical Center, and 40-hour on-site nursing care Monday through Friday allows our residents to get the highest level of care at our Autumn Wind community.
Autumn Wind offers a choice of 75 apartments, including both studios and one-bedroom units of varying sizes. Each apartment has a kitchenette, private bath with barrier-free shower, wall-to-wall carpeting and a 24-hour emergency call system. In addition there is 3 Expressions Memory care villas that can accommodate 35 residents in private and semi-private apartments.
Our community features three courtyards including a beautiful garden with a soothing waterfall and freshwater fishpond. We also have a beauty salon and private dining rooms which are perfect for entertaining family and friends.
Autumn Wind offers an enhanced service package not available from local competitors.

Expressions Memory Care :

Hospice Care
Hospice Care :
The end of life is a period in which every individual deserves compassion, dignity and respect. Our Hospice services provide each individual with the medical care and emotional support they need to make the most of the time they have left with the ones they love.
Our focus is to help relieve pain and other symptoms so residents can live comfortably and alert while maintaining a reasonable course of medical care 24 hours a day. We work cooperatively with the resident and their family to coordinate the delivery of psychological, emotional and spiritual support consistent with each individual’s beliefs and wishes.
We honor living wills, advance directives, and we welcome the involvement of outside counselors, therapists, social workers, aides and volunteers. Visitors of all ages are also welcome to visit residents under hospice care at any time during the day or night.
